medication management

Medication management is the process of organizing and tracking a patient’s medications to ensure safe and effective use. For persons 17 years of age and younger this service provides assessment, management, treatment planning and medication training support.

Service

Who it’s for

Children, youth, and adults experiencing a behavioral health crisis who are not currently admitted to a crisis stabilization unit. Services may be provided in extended observation or crisis residential settings when appropriate.

What we do during a crisis

  • Risk assessment: Evaluate risk of harm to self/others and immediate safety needs
  • Emergency care access: Crisis screening and rapid response; telephone access; urgent and routine care linkage; coordination with emergency medical/crisis services
  • Behavioral skills coaching: Brief strategies to reduce distress and manage symptoms
  • Problem-solving & reality orientation: Help identify symptoms, triggers, and next steps
  • Support & stabilization: Clear structure, instruction, and emotional support to cope with immediate stressors

What we document

  • Risk of suicide or homicide
  • Substance use concerns
  • Trauma, abuse, or neglect indicators
  • Outcome of the crisis (e.g., safety plan completed, transported to hospital, staying with a supportive adult and next-day appointment scheduled)
  • All actions taken (interventions, safety steps, referrals)
  • Your response (and, when appropriate, input from a legally authorized representative or family)
  • Pertinent events during service delivery
  • Follow-up plan, including referrals and contact schedule

Cost & access

Medicaid accepted. No out-of-pocket costs for Medicaid-eligible families.
